It is our pleasure to assist you in maximizing your insurance benefit by completing your claim forms. As a courtesy, in addition to filing the claim, we will initially ask you only for your estimated co-payment. Please understand that this is only an estimate, and is based upon the information available to us.
We are participating (in-network) providers with Aetna PPO, Cigna Total / Radius, Cigna PPO, Delta Dental Insurance PPO / Premier, FCL / BlueDental Blue Access Max, Solstice PPO, Sun Life Financial PPO & UnitedHealthcare PPO.
Note: River City Dentistry does not participate in any HMO, Medicaid or Medicare plans.
Most patients have policies which gives them the option to see the provider of their choice, whether they are in-network or not. We are happy to call your insurance company and check on your benefits for you.
The financial obligation for dental treatment is between you and our office. We will assist you in any way that we can in filing your claims; however, your insurance coverage is a contract between you and your insurance company. Your estimated co-payment responsibility is requested at the time services are rendered. Once your insurance carrier has paid the claim, any difference will be due upon receipt of our statement. If for any reason, we have not received your insurance carrier's payment after the claim has been submitted, the remaining balance will be due and payable by you.
